ADD testing is generally done at a clinic that does testing. It involves more than just answering a few questions. The tova test is generally used for ADD testing and is done with a computer like screen with special flashing red dots that you have to respond to via a signal button. There are several places out on the web that have questionaires regarding ADD but without official testing and a concrete diagnosis from the person that preformed your testing I would be hesitant to lable someone as ADD.
