I don't know of any but remember that there is no one test for ADD. If you are interested in screening questionnaires, I might suggest the local bookstore, or Amazon.com. They will have a good selection of books about ADD. I would recommend the book Driven to DIstraction by Edward Hallowell and John Ratey. They are both psychiatrists with ADHD, and emphasize the "positive" side of ADHD. The book pertains a good deal to adults but there is also information for children too.
