Hi Craig, i am from ut. and i go to a place similar to what you are looking for. They go off a percentage of what you make per month/hour etc. and it is very cheap. I don't know if this will help, but have you checked with your local Human Resources Dept.???? I think they will be able to point you in the right direction.
