My mom has had dementia for about 8 years and it has advanced to where she can't do much for herself and she can't carry on a conversation. I have noticed that at night when she is tired, or during the day if she hasn't taken her hap, she will start to talk to the thin air and sometimes yell at no one, or yell at someone who is not doing anything to her. My mom is not in pain, she just gets moody and angry. It sounds like your mother-in-law is in a further stage, so it's hard to say what is happening with her. If it were me, I would at least make sure she is getting her nap time. A couple of things that have helped my mom noticeably is Aricept, taking anti-oxidants such as Vitamin E, raisons (which has anti-oxidants as do fresh blueberries), Ibuprofen (which is an NSAID and is rumored to help slow down Alzheimer's) and getting a lot of sleep and enough protein (for her brain to function better).
