Have you checked your boy for lead poisoning? My boy was postive for lead, and one of the first effects of chelating him was his tolerance for pain decreased. This reduced the severity and frequency of his head butting and hitting. Turns out one of the chief side effects from lead exposure is reduced ability to feel things, which made him more likely to head butt as it didn't hurt him.
