I had several of your symptoms before I had neck surgery on c5 and c6. Headaches, fatigue, muscle pain, among others. Its hard to say exactly what could be going wrong but obviously there is something going on. If you have been going to this doctor for that long and you still have symptoms I would also say its time for a second opinion.
I know its hard to get across to most doc's that we are in real severe pain but you know you are. I think in most cases you would have to have a diagnosis to be able to go to a pm doc.
Please try to get a second opinion and if not get a full spinal MRI.
