If the state hasn't officially yanked it (or suspended) yet (sent you a notice) then put it down.
Meanwhile, apply for the "state ID" not the Drivers license at the City/county office to get it rolling if you don't expect to be able to drive.
If you expect to be questioned on that issue, come up with a pre-response. Such as you choose not to drive. However, do NOT bring up medical issues in the interview process. You don't want to give them reasons to turn you down.
