Just a guess here but I would think that the clinical trials revealed that getting the vaccine in the deltoid muscle was more effective than giving it in the fatty tissue. Since it's given to school age kids, often times in mass vaccinations, it's also probably easier to administer in the upper arm. As in all vaccinations, some may experience varying degrees of soreness while others none at all. Lots of times it depends greatly on whether the person is tense when they get the shot and also in the way the person administers it.
