I don't know about the severe pain, but my mother in law found her nodule through vertigo, (dizziness) from a nodule, and that is a very large nodule, there might not be room in your neck for it, it may be compressing.

A nodule that large should come out. The FNA only gets a sample of cells---not all, so it's not definitive anyway, I don't see a reason to repeat it, I think it's better to just get it removed, especially since it's so large and hurting you, no reason to keep it.
