This may sound crazy, but has your dentist checked you for bruxism? I had tremendous pain in my neck and shoulder years ago-turned out that it was a result of grinding my teeth at night. My sister complained of the same thing and I suggested she talk to her dentist-same result. Mouth guard in at night, no problem since! If they aren't finding anything else, it might be worth checking.
