Sorry to hear about your dad.

From what I gather, the doctor's recommendations are consistent with the current guidelines. A carotid endarterectomy is indicated in patients with 70-99% occlusion; nothing I've read goes to 100%. Although he is relatively healthy, given his age surgical intervention is probably more risky than not operating. A second opinion is always a good idea though, and it may give you more peace of mind. Good luck!
