A lot of your symptoms could be caused by depression. I suggest you talk to a doctor and therapist about your concerns. I think being able to describe your symptoms as need be to your doctor is more important than being able to do a self-diagnosis. You sound very self-aware of what has been bothering you so I think a good doctor should be able to help you out.

I think it's especially important you talk to someone as soon as possible about how little you value your life. Even though you don't have suicidal thoughts, it's plausible you may engage in self-neglect when you don't care if you would only have a few days left to live. And please note, if you don't seek help you may start to have suicidal thoughts if your symptoms worsen or don't improve.
