I have heard of those RFC forms but I do not know if it is standard practice to send them to your DR. A lot of times the DR's can fill paperwork out electronically and it is sent directly to DDS. I agree that it is important to have DR's supporting your claim of disability, that makes it a lot easier for a approval. Id social security does not have enough information to make a decision on your claim they will send you to one of their own DR's .
