Glad to hear that you're getting some help from your doc. Now, my son had a CT scan a few months back and his doc said that you couldn't see the joints very well with this, they were looking at his sinus area, too. The doc said that he needed an actual MRI of the joints themselves. Since he's had an MRI of the joints and it came out normal, the tmj doc seems sure that it's muscle-related.
As for nasal steroids, I've taken them before, but my doc said, no longer than 10-14 days. Apparently, we start to rely on them for congestion, and if they're taken too long, you'll end up with worse sinus congestion than you started out with.(that's what the doc told me, at least)
