ATLANTA, May 21 /PRNewswire/ -- It is estimated that Americans will spend
$10 billion this year on prescriptions for Proton Pump Inhibitors (PPIs).
However, a new study being presented today at Digestive Disease Week shows
that 40% of GERD (Gastroesophageal Reflux Disease) patients taking a single
dose of the most popular PPI, Prilosec(R), experience breakthrough heartburn
symptoms at nighttime. More importantly, this study revealed that an evening
dose of the new over-the-counter (OTC) combination heartburn remedy Pepcid(R)
Complete is faster and more effective at suppressing nighttime stomach acid
than a second dose of Prilosec.
"Physicians often increase the dose of the PPI to control breakthrough
symptoms," explained study author Frank Lanza, M.D., Clinical Professor of
Medicine Section of Gastroenterology, Baylor College of Medicine. "However,
according to this study, the optimal treatment option for nocturnal acid
breakthrough is the new OTC combination product, Pepcid Complete rather than a second dose of Prilosec. It will also be more cost-effective."
Researchers at the Houston Institute for Clinical Research and Merck
Research Laboratories conducted A Four-Period Crossover pH Probe Study
Comparing Prilosec(R), Pepcid(R) AC 10mg, Pepcid(R) Complete and Placebo in
Preventing Acid Breakthrough in Subjects. This crossover study compared an
evening dose of Pepcid AC or Pepcid Complete with that of Prilosec and placebo in 24 subjects taking Prilosec 20 mg once each morning.
Each subject received Prilosec 20 mg once each morning throughout the
entire study beginning seven days prior to the randomization period. Each
subject then received a single evening dose in random order of each of the
four treatments (Prilosec, Pepcid AC, Pepcid Complete or placebo) with a five
to seven day washout period between each treatment. The dose was administered three hours after the evening meal. Gastric pH was monitored for 24 hours, beginning one hour before the evening meal. Key findings include:
* Suppression of nocturnal gastric acid secretion was superior to
placebo with an evening dose of both Prilosec and Pepcid AC. However
acid suppression with Pepcid Complete was superior to both Prilosec
and Pepcid AC.
* It was thought that an evening dose of an H2-blocker might render
Prilosec less effective in the morning. Thorough monitoring revealed
that acid control was not different the following morning in the
group receiving the evening placebo vs. the group that received
Pepcid AC or Pepcid Complete.
* The cost per dose of Prilosec is $3.60 versus $0.35 per dose of
