Hormones are the problem.

Not for everyone I'm sure, but for many. Based on the fact that quite a few women I know seem to have break outs around certain times of the month, and it is also something we all know tends to hover around puberty quite a bit, for those of us who are older, it would also explain it. I suggest going to an endocrinologist if you have the financial means to, and getting your hormone levels tested.
