That is a really good question. Unless you get daily bloodwork done on a regular basis (which may be costly) while trying various treatments will be the most accurate way to know from hormones, diseases, etc.
I think one of the best methods is to test certain foods and/or drinks and their effects on your skin. This you can directly test by keeping a journal and writing down all the details. Seeing what foods wreck havok on your skin will be body chemistry related and will not be a one formula for all.
Certain foods affect hormones also, i.e. eating a lot of pineapple daily will help to induce more testosterone circulating in the blood, whereas eating a lot of soy products will induce more estrogen to circulate in the blood.
There are a lot of factors that can affect acne as you lined out, but I think it is best to start with diet to see what foods induce acne and/or make it worse. Then from there can you test the amounts you eat of these foods to see if that makes a difference and then choose foods that will keep your skin acne-free.
It is a tedious but necessary process for long-term acne healing.
