The symptoms could be caused by many things. You say you have always had trouble concentrating but its gotten worse? Symptoms must have been present before age 7.
I think that you should go get blood tests, urine tests, and a sleep study to start, along with a full physical.
After that if you get a clean bill of health you may want to talk to a psychiatrist.
In the mean time some things you can do are make sure you walk or run for at least one hour every day!!! This is VERY important. It sounds like its not a big deal but believe me it is. If you do have ADD there are many nutritional treatments that can help. L-Tyrosine is cheap and effective for quite a few with problems such as depression, ADD, fatigue and more.
