Do meds need to be adjusted now and then??

Wanting to know if anyone else has been through this and what suggestions anyone might have...

My son 8yrs old was diagnosed in K-garden. Put on Ritalin 2x's a day, worked well...but we tried a timed released Concerta due to the ups and downs mid-day and so we didn't have to medicate in school. (can't remember the dose back then) but it didn't work as well as Ritalin 2x's a day did. Then..after seeking a behaviorial specialist we upped him to 54mg. of Concerta and it seems to work well.. He is very well behaved!! in and out of school. Grades took a tremendous jump forward!!! He still struggles in READING/Comprehending what he reads.... After doing very well in 2nd grade no complaints at all from the teacher!! in anything now in 3rd grade his teacher is really really tressing he isn't understand anyting he reads.. Well I have insisted that the school give him his tests orally!! and he does really well... We would study and study and he would barely pass so his teacher agreed with me that this is definetely what he needs if really makes a difference. His teacher has questioned that maybe might he be on too much meds??? She says that when they are in reading circles or someone is in front of the class reading a story he seems to be in lalalala land??? We read with him at home, we bought hooked on phonics to do... He never complains about doing his studying but why don't he get it???? He can read, he isn't fluent about it but he knows how he just don't understand at all what he is reading.. ANyone out there have any suggestions for me, we are going to doctor on Tuesday next week to see if possiblably he is overdosed?? and see what suggestions his doctor has. Do meds need to be adjusted occasionally?? I would love anyone elses input..... Math/Science he is at the top of his class - story problems he does ok...but basic math facts no problems...
