Concerta and Stratera in combination

I would like to get some input on using Concerta and Stratera in combination. I have tried it with good success with my son but want to learn of any potential problems.

My son exhibited symptoms of ADD in preschool. After a very through evaluation by a specialist we tired Ritalin. Through middle school we had many struggles, but managed to get mostly B's although each year we had to increase the dosage. We starting using Concerta in 7th grade and no longer had to give a midday dose. However by the end of 8th grade we were up to 126mg a day and having trouble in that he was not hungry until 10 at night.

My Son really wanted to play football in High School, but we knew he could not do it without eating during the day. We started him on Stratera the last two weeks of 8th grade and seem to have fair success. He was definitely able to eat, but I would say it was only 80% as effective as Concerta. Once classes started we had struggles keeping up with math and biology. Because football was so positive for him even though he was only a fair player we kept him on the Stratera. However at the same time we started to notice that he was becoming increasingly depressed. After a serious downturn we started him on Prozac to good effect after about 3 weeks.

However he started to have increasing problems with drowsiness that was becoming a major problem. Knowing that both Stratera and Prozac were associated with drowsiness I started to look for a better solution. From the literature I knew that Concerta works by adding neurotransmitters to the brain and Stratera works by keeping the neurotransmitters you naturally have from being absorbed as quickly. It seems to me that in combination the action of Concerta would be more pronounced.

I tried this with my son starting out with 18mg of Concerta and did not see any adverse effects. At 36mg he experienced a dramatic increase in concentration and has done some of the best school work we have ever seen. The really good news is that he is still hungry at lunch. I have lowered the dose of Stratera to 40mg and so far he seems to be doing very good with his concentration. At the same time his outlook on life has also improved dramatically and we are trying him on half the dose of Prozac at 10mg.

We have a regular appointment with his doctor next week and will go over this with him, but I want to get any input I can as I have not found any literature on this combination.

There are several of us here who have kids taking a combination of Strattera and a stimulant. My 13yo son started on Strattera last summer and over the Christmas break we began slowing adding Metadate (a long-acting methylphenidate like Concerta). We are extremely pleased with how he is doing on it.

Like your son, he has a better appetite on both meds than on one or the other alone. He is also sleeping better now than he was on Strattera alone.

(As an aside, your son was taking 126mg/day of Concerta??? OMG! I've never heard on anyone taking that much!)
