It looks as if this job is too much for your grandmother, and she is taking it out on him. he must be quite a handful, just the incontinence alone is a major problem. He ought to be wearng Depends, but I know how hard it is to convince the patient to do so!
I am 66 and caring for an Dementia patient ( mother) 96 years old, and I'm still working full time. We have a Home Health Aide for some parts of the day. That is one thing you could help arrange for. There are many agencies which supply these aides. I don't know how old your grandma is but it may be physically too demanding for her strength. If so you need to help get your grandfather into an assisted living or other facility. Ask his doctor.
It is kind of you to want to step in and help them; they need you. Try to get other family members invoved also. Good luck!
