Can I switch SSRI's while Weaning off Zoloft?

I obviously am waiting for my Dr. answer but I was at 100mg of Zoloft for 3 weeks and it sucked bad. I had more anxiety than I ever had. I am trying to wean down and am wondering if I am going to another SSRI (lexapro) can I just switch or do I need to come completly off zoloft?

I was on Lexapro once and it worked but pooped out 3 years ago. Howver, I think I am going to try it again as I got good results before poop out.
