Could use suggestions on diagnostic possibilities.

Hello there,
I have posted this in the "blood" section on the message boards and have received no answers. I am looking for some help, even if someone does not have any explanation of the blood tests, I am looking for potential suggestions as to what could be "wrong" with my Fiance.

I was just looking for some clarity on a recent blood test. My Fiancé has had severe chronic back pain for 3 years now and has seen over 100 doctors with no answers. They continue to maintain is pain level at moderate by using heavy medication. While suffering from the back pain he has experienced several other symptoms, including blurred vision, heat sensitivity, muscle weakness and many other random things. His most recent doctor has decided to send him to what would be his 3rd Rheumatologist. In doing so the Dr. had him take a blood test. His platelets, white and red blood cell counts have all been fairly low for some time now, however the most recent test reflects some other high or low readings. Can anyone please help us to understand these results and offer potential routes for a diagnosis. He is awaiting his appointment with the Rheumatologist, but having done this before we are looking for alternative areas for a wider variety of answers. Thank you!

Results are as follows (anything in red was highlighted as abnormal, everything else remained within normal limits):
WBC- 3.7
Hemoglobin- 13.2
Platelet count- 103
Glucose random- 92
Bun- 11
Creatinine- .86
Calcium- 9.4
Sodium- 143
Potassium- 3.9
Chloride- 102
CO2- 32
Anion Gap- 13
Bilirubin- .4
Alkaline phosphatase- 78
Protein- 7.3
Albumin- 4.2
ALT- 26
AST- 25
Rheumatoid Factor- Positive
Rheumatoid Factor Titer- 10
C-Reactive protein- 2.26

Re: Could use suggestions on diagnostic possibilities.

Dear hawest, Has your fiancee had previous spine surgery, epidural steroids or a myelogram? Reason im asking is I have a condition caused by one or more of these things & has similar symptoms so thought that i would ask & if so, does he have an MRI report of the spine that you could post? I hope that the answer is found quickly & I admire your helping him.

Re: Could use suggestions on diagnostic possibilities.

He has had no previous spinal surgery. He had an epidural to attempt to treat the pain after the pain was present for about a year and a half. The epidural increased the pain for about 3 weeks, during that time it also caused breathing troubles. After 3 weeks, the pain returned to its prior state and his breathing problems went away. He has had a myelogram, which they said had no abnormalities. The myelogram was around the same time of the epidural. He has had multiple MRIs and Xrays and other tests done with no answer found. We do not have a copy of his MRI. He had a bone marrow biopsy, multiple different blood tests. He has seen physical therapists, chiropractors, laser therapists, and many other different specialists. There has not been any doctors able to find a diagnosis. Most doctors end up trying a few things and then give up because they feel they can no longer help him in solving his problems. Any knowledge or recommended tests would be helpful.
Because he is only 21, the doctors want to stop prescribing pain killers and have offered no other form of treatment. At his most recent appointment the doctor suggested tapering off the meds and learning to "deal with the pain".

Thank you for your concern!

Re: Could use suggestions on diagnostic possibilities.

Dear hawest, From what i have read i think it is unlikely that he has the same condition that i have because it is almost always caused by a foreign substance entering the arachnoid membrane or meninges during an invasive spine surgery or test & he had the severe back pain before the myelogram & a single ESI. However, there is a classic sign that is considered a hallmark in the condition, adhesive arachnoiditis, that is on the MRI report & it is "clumping of nerve roots" . I had 4 back surgeries, CSF leaks, dural grafts, esi's, old oily dye myelogram that caused it but i was never told my MRI results that showed it from 1988 to 2012, so i now get a copy for myself of any tests. I had chronic pain for 24 years undiagnosed so i understand some of what he is going through & i have found that trust between a patient & dr is so very valuable & developed over time & i can understand how difficult it would be if the dr kept releasing me... heartbreaking. Your fiancee is so young i will pray for him to be diagnosed & until then for him to have his pain relieved. God bless you both.

Re: Could use suggestions on diagnostic possibilities.

Thank you for your help and your concern. We will continue to hope for a diagnosis of some sort, hopefully the upcoming appointment with the rheumatologist will give some answers. It sounds like you have had a lengthy process with your pain as well. It certainly is a challenging thing to deal with.
