I'm sorry you're having to go through all this! I know how bad dental pain can be.
It could be TMJD but it could be other things. Is it possible that maybe you've had a failed root canal or maybe something is wrong with the crown?? It may be too high and throwing off your bite.
I've never had a crown so I don't know what's involved with them but my ex MIL had one that gave her some problems and then her DDS adjusted it somehow and she was fine after that.
I'd go right back to the dds who did it and get him to check it out and don't put off being seen. If you don't have ins. ask if they can work something out with you on payment, etc. Most of the time they'll do that.
I hope you feel better soon, let me know how it turns out.

Jenna,
It should not cost you ANYTHING to get this looked at and fixed. Go back to the dentist who put the crown on and tell him what is going on. Given you did not have pain until after this crown was put on, probably means that it's an ill fit. I'm not a dentist, so I don't know for sure, but it's worth your time to look into it.
Since the dentist who put the crown on was the last one to work on you, and since the pain started after that treatment, chances are very, very high that whatever price you paid for this also included basic routine follow-up care, such as periodic adjustments to fix the bite.
Call him and be sure to emphasize how extreme the pain is. If you make it sound like you think you're being an imposition and that it's a bother,... the gatekeeper (ie: receptionist) will put your appointment off, not viewing it as an "emergency" because you're not bleeding, and it isn't as if you just broke a live tooth) -- so choose your words carefully. Tell her YES this is an emergency, that you have not been able to sleep, tell her that you're taking the maximum dose of advil (or whatever pain medication you are taking), and that has you worried, and that it's getting worse, it isn't resolving itself, and that you're getting referred pain, ... and that you'd really appreciate having the dentist just take a quick look and check the bite...
As I said, he really should do this for free.
