I think keeping things out of your vagina is the main concern to prevent infection. Also, increased blood flow to the area could exacerbate any bleeding or spotting. If you feel like you still have wounds, these could open from the blood flow or even from straining. I am very physical and I don't know if I could go all the way without tensing my legs and abs. I don't think I could just lie back and not strain.
