What does "designation" mean on the new registration form? I'm trying to enlist my company in this website, so can I do that instead of listing each of our five doctors one at a time?

It's for LocateADoc.com. I'm trying to register my company for the free listing. There's a required box that says "Designation". What does that mean? Also, the "primary specialty" won't let me type anything, only the "secondary specialty". Is there a way that I can enlist the whole company under the listing instead of listing all 5 doctors seperately?
