Hi all,
I have been to see a Doctor David Miller in Sacramento. He was v good and is the first specialist to realise that I have a pyhsical problem and am not just stressed.
I am not in much pain, don't get the migraines etc that a lot of you get. My main problem is that my teeth don't fit together properly and sometimes bang into each other when I am talking. I also get muscle tension and my left TMJ is clicking etc mostly when I wake in the morning.
Anyway since the beginning of this nightmare I have been convinced it has something to do with my wisdom teeth which came in and messed my bite, leaving me unable to close my mouth where it wants to close. I told the specialist this and he convinced me to get my wisdom teeth out in the near future. He also shaved my wisdoms down a bit to allow me to close my mouth and relax my jaw in the meantime. Anyway he shaved them a bit and they felt a bit better. He also told me just to buy a ordinary mouthguard froma sports store and wear it at night. He also gave me a prescription for muslce relaxants. His theory is that the shaving and the mouth guard will let my muscles relax and then I can get my wisdom teeth out.
Here's the problem, I am so scared about getting my wisdoms out when I have problems with my jaw. I still haven't had any tomos or MRI done. This guy reckoned that it was probably displaced but that he didn't need the tests to see that.
Does anyone have any opinions on this. I am worried about using the mouthguard in case it makes me worse. He said that if that happens I have to stop using it. I am tired of all of this. I want to fully trust this guy as he has a lot of credentials etc. but I am scared that I am not getting the proper treatment and that I am just making the problem worse.

First of all you say he has credentials? He should first take xrays. mri, and
give you a diagnosis test first before going off an "shaving your teeth"
Secondly drug store "mouthguards' are the worse thing you can put into
your mouth. YOu need a specially fitted one custom made to fit your bite/
occusal problem. I would not do anything with the wisdom teeth until you
get properly diagnosed. Definitely get another opinion.

you need to get a proper diagnosis and that usually includes tomograms, mri, etc. it's important to know what's going on in the joint, and i find it strange that your dentist isnt going to do that especially since he's already started treating you without a diagnosis [muscle relaxants, shaving off your teeth, etc].
i'd get some more opinions concerning your wisdom teeth removal. i, personally, have opted to wait as long as possible to get them out.
i've only heard bad things about the store bought mouth guards -- they seem to make TMJD symptoms a lot worse for patients. imo, a splint should be made by a specialist, and i wouldnt trust something like that off the shelf.

I've read about your dentist, and he is supposed to be very knowledgeable and well respected in the field. I'm finding more and more that the dentists that are considered experts and are well respected by their colleagues, are often so for reasons other than their treatment of patients (for example, due to their publication records, how many committees they're on, etc.) In the 2 years I've had tmjd, I've seen some of the most respected dentists and they were unable to properly diagnose me.
Generally, your bite should not be adjusted until your joints are stable. Also, having wisdom teeth removed has been known to cause or exacerbate tmjd symptoms. I have all 4 of my wisdoms. I've had them for years, they came in just fine and are straight. And yet, I'm surprised at how cavalierly a number of dentists I've seen view them. A dentist who was having problems doing impressions for a splint because I can't open my mouth wide, suggested to his assistant that I have them removed. I imagine that would make taking impression taking easier, and yet there's not a thing wrong with them, and so many things that could possibly go wrong removing them. I don't mean to scare you, but you're right to be cautious. It sounds like you'd be wise to get a second opinion.

Oh goodness. This dentist shouldn't be shaving your teeth before treating your TMJ and getting your bite stable first. Before moving on to any type of permanent treatment your bite needs to be stable first and you need to be symptom free. He could do more damage than good. And those store bought mouthguards can make your problem worse too. You need a splint that is made to fit your mouth and your problems. Please before moving on with treatment with this man, please get another opinion. I'd hate to see you spend alot of money on improper treatment then end up with more problems than you started with.
