If like sharp spasms: kidney stones, gallstones.
If translucent vesicles on the skin only in that area: herpes zoster (shingles)
Possibly: intervertebral disc damage or degeneretian with herniation and thus pressure on the nerve root (with pain, numbness or tingling)...
