I am a 38 year old female with two herniations in the L4/L5 and L5/S1. My discogram showed negative for back pain. I have terrible pain in my back, hips, butt and numness in my right leg. Yesterday I had my sacroilliac joint injected. Initally all of my pain was gone, until the novicane wore off. I am now convinced that it is my si joint and that i have been misdiagnosed for almost 1 year. What can I do to fix the problem and what kind of doctor should I go to?

I have about the same problems as you have. I am a 33 year old male that has been suffeering with this God awful pain that anyone could ever imagine and then multiply that by 100x and that is the way my back has felt like at times.
I have a problem at my L5/S1 vertibrae called Spondililothesis (I think thats the way you spell it), anyway, it is that L5/S1 has a slippage up to 3 1/mm. From L5 and up has shifted forward and S1 and down has shifted towards the rear. Whenever I bend forward any llittle bit the slippage increases to about 4mm.
I have been to 20 differant Doctors, Specialists, a Neurosurgeon, a few Family pratice docs, a couple of Chiropractors, Sports Medicine, A Piece of Crap For A Pain Clinic at HealthSouth in Birmingham, AL., a PHYSCO Doc, and now have a wonderful PM Doc that has helped me tremendously, a Nurse Practictioner that is in with my Pain Doc at U.A.B in Birmingham, Al.
I have had to quit work back in August of 2002 because of my Increasing pain. I had resulted into buying pain meds off the street because the docs would not prescribe me hardly anything at all. I finally got in with this Pain Doc back inn Sept. of 2002 where I was put on Methadone 10mg 2x a day. After 8 months of that dosage I was then raised by 1 more tablet a day making that 3x a day. I have used Lidocaine 5% adhesive patches on the tender areas. I have now developed all of the symptoms of having Fibromyalgia (FMS). I have an appt. in 3 days with a doctor that is a specialists with FMS and treats it with herbs.
I am hoping that my ppain doc will start me out on the Duragesic Patches in a month or so. My Neurosurgeon has talked me out of surgery 3 or 4 times WARNING me that I would be in much more pain after 75% of all back surgeries, but yet he would not prescribe anyting over a Flexiril for pain.
I have cut 95% of my life away from the outside world because I can't get out of bed for 2-3 weeks at a time. I would suggest having an MRI done on your back, maybe a bone scan, X-Rays standing straight up, bent forward at an angle then at 90*degrees and then bent backward.
I hope the best for you and always be honest with your docs don't be afraid to ask for something by name without feeling guilty. I will try to keep you up to date on what all of my diagnoses turn out to be.

Dragracer n pain, Thankyou for your response. It sounds like you are worse off than I am. I have a question. How can you tell if you disc is moving like that? I wonder if mine is. What is methadone? Is it an antinflamitory or is it a pain killer? I was on the pain patch. I thought it was a joke. It just put me into a coma. I was very crabby, tired, and the pain was still there. Today I actually feel Pretty good. This is day 2 after my si joint injection. Did you try that? Keep in touch.

Colleen ,Sorry to hear you have been going thru this the injection may help you it doesn't always take effecet right away.Are you scheduled for more injections I know most are in a sries of 3 if it seems like they maybe helping.I hope this is the answer for you and that you continue to improve.

keep us posted on how you do with this injection. You mentioned the SI to me before... and I told you that the doc has checked me several times. I don't know if I mentioned my tailbone.... and that I was getting an xray as it's never been done? Well... I went yesterday and had 2 lying down and 1 on my left side. The Tech came back in and I said "I know you can't tell me anything, and I'll pretend I don't hear.. but, did you find something funny?" She said "well, that's why we're taking another one on your back to get a darker pic" http://www.healthboards.com/ubb/rolleyes.gif So... I think they've found my tailbone pain problem! ;) I'll call back this afternoon.
What I wanted to say about the injections, before I started babbling, is that I had a series of 3 as Linda (jdlmc) mentions. My first 2 were Rt Transforaminal which didn't do anything for me. The 3rd one was Lumbosacral.. and it actuall DID numb my tailbone for a couple of days.
My understanding is that when these injections help - but only for a short time... that it's been a diagnostic shot vs a therapeutic shot and you are now left w/ the cause of your pain.
What is your next step? When do you go for a follow-up? As Linda said, sometimes it takes a week or so to feel relief. Sometimes, it's immediate and then wears off. Be sure to tell your doctor this as it c/b useful in your next step. :)
Good luck and let me know how you do. I am following your case as it's so closely related to some of my 'stuff'!

5/9 - Discogram - shows Tear L4 w/ significant leak onto nerves, DDD
Performing doc oversedated, False Negative (pm doc & I agree)
New Consult w/ Dept Head Cleveland Clinic 7/8 to discuss possible new discogram/IDET candidacy
7/8-Doc ordered Facet Joint nerve block for 7/29 to rule out/identify as pain generator. If not, new discogram.
Also, thinks tailbone prob is dif issue - wants x-ray - THOUGHT I had one - didn't!! They are ordering.

It is great to hear from you all. I think you are more concerned than my doctor about me, and I am grateful for that. It is so nice to know that there are others like me out there.
I go back for injection # 2 on the 21st of July.

I live just outside of Washington D.C. I decided to find a new "primary care othopedic". I was getting no where from my other one. He kept telling me to learn how to deal with the pain. So tomorrow I am going to a new one at Georgetown University Hospital. I have always had good luck there with everything else so I may as well give them a shot with my new challenge.

Colleen,I know what you mean about people here caring more then the Drs.I get so mad and frustrated when each Dr just passes you throgh like a number on an assembly line,I don't think any Dr but my GP ever really listens to me and that helps but not my back,everytime I get all excited about seeing a new Dr get my list and leave so D*** let down I know that to all of them our cases are not unique and something they haven't heard over and over agian it surly seems for what they get paid they could at least fake caring!!!
I've had explained to me on so many occassions that dealing with the back is one of the most difficult to diagnois,well don't we all know that,they say it is a process of starting where they believe the source of the pain is and begin elimanating amd then as they find and solve one problem there is usually another underlying that one.
I have been going thru this for 4 yrs I was put on a back burner over 2yrs ago when they found several sources of my probems because WC and private Ins. had to fight about who was paying I have lost my WC case and now the pre-existing time has lapsed on my ins. so now I am about to start over again and I know there is a lot more damage then was found back then,I have only existed on pain meds for these 4yrs and I am looking foward to maybe adding a intelligent thought to these post.
Sorry to rattle but it has been the caring people here that has made it possible for me not to just throw in the towel and quit I found this site by accident one day when I didn't think I could go on.
So you have a great source here and you live in an area with some of the best profeessionals in the country,so I think you are on the road to success!!

I was having debilitating leg pain and never connected it with SI until I stumbled onto it on this board. I am working with a physiotherapist and booked for an appt with a "physiatrist". I wear a SI belt, what a lifesaver! The leg pain is almost gone and now I have lower back pain. Apparently this is good as it it is "localizing". You guys sound much worse than me and I didn't think anyone could ever feel such pain as I had! It was ruining my life. The physio is teaching me "coping techniques" for work where I walk and stand a lot.
Some days are very discouraging. I guess this is something that never goes away. You just have to live with it. Well my garden is overrun with weeds, my dog is getting fat and my husband doesn't understand why I don't want sex right now. Man, those buttock pains are killer.
The physio says it's important to keep exercising, jsu keep changing positions and exercises. Sometimes the pain is so bad exercise is out of the question. I'm flat on my back with my knees up (and not for sex!)
It's consoling to know that others feel this pain. It validates it somehow even though it's not so great for all of you. It scares me that it may keep getting worse and worse as I read in yur posts.

Colleen Good Morning!Hope you are feeling well this morn.Yes I have gone through this for 4yrs.I will put my sig.at the end so you can see what I've been doing these yrs.I'm afraid I got caught in the middle and not beign knowledgeable about what was going on with me I let thing get out of control.I haven't been back to work since 6/7/03 when I had to leave early as the pain took over completly and since I haven't even been able to even get the milk out of the fridge and I'm sure I won't be going back to serving,I was to old to be doing it anyway but it was all I have ever known and you know what good money it can be,it's tearing me up trying to get use to not having money every day and knowing I never will again,most people don't understand how very demanding of a job it is physically and emotionally ,you asked how I did it I over medicated every day I worked and suffered for it every min. I wasn't working and the sad part is I kept telling the drs I was doing this and all they ever said is work it's good for you and do it as long as you can well I did and now I am paying the ultimate price for it!!
I have always been a hard worker and I loved it and I think in my mind I just could not believe this was happening to me what a fool I have been.
So I hope you don't push yourself over the edge like I did I wish I had paid more attention to my body and had more faith in myself then to believe others knew what was best for me.
I hope that you find the answers and get relief soon.

NO relief asked referrel to PM Dr.started PM 8/25/99 mri said to be normal slight bulge L4/5 all request for further testing denied.
8/01/01 I paid for discogram from local orthepic surgeon after given 2 4mg. shots morphine discogram found to be normal
WC approved 3 epidural inj.NO HELP
10/03/01 started treatment Cl.Clinic on private ins.
MRI:Cl.Clinic L4/5 disc bulge w/neuroforaminal impingement
X-RAYS:Cl.Clinic L5/S1 spondylothesis
3 more epidurals no help
2/01 discogram Cleveland Clinic:L4/5 disc bulge contrast moved towards posterior asppect of that disc w/midline fissures in the medial annular fibers disruption
L/5-S1 level also abnormal.
Update;at that time I was prescibed a spine cath idet/denied WC denied privat ins.
since have been living on pain meds prescribed by the approved WC PM Dr.
vicodin 10/660 4xday
neurontin 400mg.3xday
soma 350mg.2xday
oxycontin 40mg.2xday
No further treatment then meds everthing denied by both ins.
Pri.Ins found out going to Cl.Clinic because of Wc accident refused pay ment for any med. bills ended up in bankrupcy court.
6/14/03 pain worsened and many other comlications.
6/21/03 ended up going to Cl.Clinic ER was a bad nightmare,after being there for over 24 hrs. was released at my demand
X-rays showed DDD and arthritis at 2 or 3 levels.;diagnosis upon release STRAIN/SPRAIN
7/1/03 mediation to prepare for trial by jury 12/17/03
7/8/03 WC hearing to discontinue all meds,medical treatment

Hi Linda, wow, what a story. I hope that wc is covering everything now. you deserve it.
I finally got diagnosed with SI Joint disfunction today. I will wear a belt, get therapy and one more injection. Hopefully I will get relief. I have two little kids that need my attention, I need to be off the pain meds.

Colleen,I am so happy you have your diagnosis and i feel confident that things will be better for you soon,do what you are told and please pay attentopn to your body I know what you mean about taking care of children I was my 3 childrens sole support growing up, Do you know how you injured yourself? I know a lot of women that have made serving thier livliehood and are now paying the ultimate price for it and of course there is those that it never bothers but they aren't the norm. I don't believe in if I could do it over again,but that is the one thing if I could do over I would have found an occupation that wasn't so physically demanding!!!Now I am going to be 50 and don't have a clue as to what I will do,my husband makes decent money but we have made our lifestyle that of a 2 income family and no longer have 2 incomes.NO NO NO WC does not have to pay a dime more for my treatment medication nothing they are off the hook they had offered me $12,500. to settle and as of now I have refused it my attorney is waiting for my new diagnoisis when I start going back to the clinic next week to see if we can get the settlement up any ,thier getting off real easy considering I will never have another painfree day!!I just want it over with them and never have to walk in to that resturant again!!I cant deal with the stress of the fight any longer.Once again I am so happy for you and will pray that you are soon pain free and can enjoy your life with your children they grow uo so fast!!PLEASE let me know how you do with you treatment!!!
