I am 47, weigh 260 lb, height 5'6".

I need tips to lose some of this weight. 7 years ago I lost 52lb with the help of appetite suppressants and eating less than 800 calories per day. I maintained that for a total of 8 months, then just put it all (plus more) back on. I need to lose weight, my knees ache (at times I can hardly walk), my back aches, my blood pressure is through the roof and I can't keep up with simple household tasks.

I know I need to eat more often as I don't eat breakfast and rarely eat lunch (I have always been like this). I have always had terrible trouble trying to eat breakfast, it makes me feel nauseas. What can I try?

I know I need to increase my exercise but I find it causes too much pain even to go for a 5 minute walk. Any suggestions?

I can only suggest some research look under other threads here on yo yo dieting and need to lose 100 pounds and other threads I have posted, first of all 800 caloires diet is to low, first of all the world health organization says that at 900 caloires the body is dying,

you are dying and your body is trying to keep you alive by restoring weight and slowing you down and increasing appetite so you will not only eat enough for immediate needs but to prepare for a future famine, 800 caloires a day is a severe famine short of eating nothing at all.

you body has been forced to cannablise it's muscles, organs as well as fat. I bet your muscle to fat ration is large, meaning more fat than muscles overall. you have stepped into teh food jail as it has been called.
