I have been drinking heavily for about 7 yrs. I got an ultrasound back the showed diffuse fatty infiltration (which I expected), but also showed focal sparing at the gallbladder and periportal areas and focal nodular parenchymal changes in my right lobe. Flow through the portal vein was normal. What does this mean

Did you have a blood test and if so what is your AST, ALT, LDH, ALBUMIN, Total Bilirubin and ALP. This could give you some insight.

I didn't have them drawn this time, but I had elevated liver enzymes in the past. I expect them b/c of my drinking, I'm just wondering if cirrhosis has set in yet.

A blood test in March disclosed for the first time an elevated ALT . I'm certain this is the result of several years of heavy drinking. This is what I did:
(1) I do a mild liver flush for a week each month: juice of one lemon, 4 oz aloe vera, olive oil, liquid garlic extract, liquid milk thistle, liquid liver enhancerwhich contains many ingredients which include the mushrooms mitake, shitaki and reishi.
(2) Several times a week I juice using danellion, beets & carrot. These are excellent juices for the liver. You can add apple.
(3) I take large amount of supplements (vitamins, minerals, trace minerals).
(4) I take two tablets daily of a liver cleanse which contains 18 ingredients. I also take artichoke tablets and schizandra tablets. Each morning before a meal I take 400mg of SAMe excellent for liver problems.
(5) Reduced intake of saturated fats.
Whatever you do don't depend entirely on doctors. Your health is your responsibility. Do lots of research. Knowledge is Power.
I plan to take another blood test in March.

I've stopped heavy drinking. Occasionally I have a glass of wine with dinner. My ALT in March was 48 (range is 0 to 40). Drank heavily for about three years but not every day. However, while drinking I always took mega doses of supplements and took milk thistle capsules. The other 5 tests for the liver were normal. While drinking I was abusing my liver but not completely ignoring it. I'm now heavily into juicing and liver flushes plus take supplements especially for the liver. Hopefully, I'll get ALT below 40. Prior to March ALT was never above 30.

Sounds like you're doing the right thing!! My ALT is in the 10 - 24 range. It should be WAYYYYYYY higher.

I heard if your liver is fatty from heavy drinking, after you sober up, it goes into remission and becomes completely normal again. A fatty liver does not always turn into cirrhosis.
I've been drinking heavily for a while now, but am now letting up, due to concern about my liver and other problems. Last time I had my liver checked, it was normal!
