Advice please?
On ultrasound in May I had a complex ovarian cyst diagnosed suggestive on endometrioma (had endometriosis 25 years ago). It was still there in late July and had grown by 1/3 but is still small, only 1.5cms. CA125 in May was 14. I am not menopausal, am 48, had fertiility drugs (Clomid for at least 12 months in 2 separate bursts) but had no children. Never been on the Pill. I experience ovary pain for around 2 hours a day, spotting etc
I would like it removed, does anyone else have a similar story?

Hi, How are you? How did they diagnose your complex cyst as an endometrioma? Did you have a CT Scan? I had an ultrasound on 8-19-04 and a complex cyst was seen and now they want me to have a CT Scan on Tues. I am scared to death! I also had some fluid in my abdominal cavity when I recently had gallbladder surgery and now it has come back. The fluid tested negative for any malignant cells and I am trying very hard to think positive because my surgeon didnt see anything suspicious, just the cyst. Wouldnt he know if he saw cancer? I am curious to hear about you complex cyst and what treatment you have had.

I am seeing a surgeon in a week's time to schedule removal. The ultrasound is pretty good at determining whether it is benign or not, but it is not 100% until they operate and test it in the laboratory.
I think there are signs that jump out and say - "This is malignant" but some seemingly benign looking things on ultrasound have obviously shown malignancy so they can never be 100%. At worst, i have something of low malignant potential (borderline cancer) because I have had it for at least 3 months and it has only grown a bit.

I had a cyst on right ovary found during lap removal of endometriosis last year. The docs said it would probably just disappear on its own and never mentioned whether it was "complex" or not. I have no children, 40 y/o and have been on BC off and on for last 24 years. Have now got mirena iud which has left me with light bleeding/spotting all the time. Ovary pain, like yours, persists for about 2 to 3 hours a day along with really low back ache. It really knocks me off my feet now as it is getting progressively worse. I am now in process of trying to get my doctor to refer me to the gyno again. I am sure he will but I have a hard time dealing with a lot of negativity. They thought the mirena would be the answer to my problems but it has just traded one bad situation off for another.

Gosh, I have the Mirena too - for heavy bleeding. I have had it for 12 months for endometrial buildup. I had a time of about 4 months when I had got over the spotting and thought Wow! But then I started spotting again. I had one Ultrasound which showed three normal cysts (ie fluid filled and they will likely disappear) and three complex ones (internal structures) that may not disappear. On a followup US all but one complex cyst had gone, and I had a couple more functional cysts (that will go away)
I think the Mirena encourages these cysts because the progesterone is not high enough to stop the follicles forming, but is too high so the egg cant break out. So a cyst forms.
Do you have endometriosis? I had it in my twenties and am now 48. No children by birth. Drs think it is an endometrioma but have now referred me to a surgeon to get rid of it. I have a history of ovarian cancer in the family so I am not taking any risks.
I reckon we have very similar stories, dont you?

Before i had mirena i had probs with severe flooding but the worse thing was the pain. I used to pass out. Anyhow it was really interferring with everyday life so that was when i approached the docs and they suggested mirena. I also have complications with vonWillebrands blood disorder. I am now in more pain than i was before and for a lot more time too so i really want to try something else if there is anything. i am seeing the nurse for a smear test and check up on the mirena next week and will follow that up with a visit to the docs so will keep you posted on what happens next.
