Hi all,
A friend of mine (male, 45, physically active, non-smoker, excellent health) just had a comprehensive physical exam with fasting blood tests etc.
All blood results were considered "perfect", except his serum glucose was 145 and the doctor wants to re-test.
Is the normal ref range 70 - 110 mg/dL? And what may 145 indicate?

Normal is 100 or less. From 101-125 you are considered to have glucose intolerance/insulin resistance (pre-diabetic). 126 or greater on two fasting blood glucoses and you are considered to be diabetic.

I just found out it was a non-fasting blood test, but still, 145 is suspiciously high wouldn't you say?
So he'll get a fasting re-test and we'll take it from there. I know very little about diabetes, but he's not overweight, has no family history of the disease, and is very physically active and athletic. Asymptomatic at this point, but is it true that one doesn't necessarily have to have symptoms in the early stages?

It can be suspicious, or it might not be...suppose your friend had just ingested a significant amount of glucose prior to the test--one possibility, for example, would be that a 32 oz gatorade had been consumed after a workout. There is an oral glucose tolerance test that examines the response and if 75g of glucose is administered, then the ranges are: fasting (60-110), 1 hr ( up to 200), 2 hr (up to 140) in mg/dL; however, 140-200 mg/dL at 2 hr indicates impaired glucose tolerance.
The second question, from my personal development of type II, I didn't associate the tiredness as a symptom until after the diagnosis in 2000. As far back as 1997, my fasting was 124 mg/dL when tested. The frequent urination did not really occur, for me, until my fasting levels were in the 300+ mg/dL range.
Finally, going back to the first question on a non-fasting test. I have experimented with sugar loads, now, with my diabetes well controlled by current weight and exercise and I find that I cannot make it exceed 125 mg/dL, regarless of what I eat.

Hmm..your personal info on Type 2 is really interesting, plus thanks for letting me know more about the OGTT.
My pal is returning from a snowboarding trip tomorrow and will be setting up a re-test. I hope it was a sugar overload/"Gatorade" deal and only temporary, but we'll see.
