Hey All,

Just a quick question, I am taking methadone 20mg 3x day for pain. Yesterday I had to have my prescription filled at a different pharmacy because I forgot about the Memorial day holiday and my usual pharm will be closed and I would have run out.

Anyhow, they gave me MethaDOSE, which is a generic. I called the pharmacist, who told me it's exactly the same as the methadone HCL that my usual pharm gives me.

Well, I took it this morning and it felt like it didn't help my pain as well as the methadone HCL that I have been using. I've researched it on the web and everything I've found indicates that it is the exact same thing as methadone HCL, but I've heard from some of you and elsewhere that generics can be as much as a certain percentage off and still pass FDA approval.

Has anyone been in my situation before? Anyone taking methadone HCL and switched to methadose found any differences?

Lets just say this...You've NOW learned your lesson! STAY far away from MethaDOSE! No joking. I think if you've never had methadone before and start by taking Methadose, then you'll be fine cause you never knew the difference. But in your case your used to Methadone, which I'm assuming is the Roxane Pharm (54/142). Methadose is made by Mallinkrodt. And everyone who has switched to Methadose has said the exact same thing your complaning about. Quality, Quatlity, Quatlity! It just doesn't stand up to the Roxane version. Don't know why...maybe it is not as much methadone per pill as the others? Dave might have that answer (shoreline). My mom just made that mistake and she filled her methadone as methadose, she was VERY unhappy. I've heard this situation before, so when I 1st started taking methadone it was the 1st question I asked the pharmacy...'What company makes your methadone', they said Roxane (althoug I get the 40mg dispersables), so I said 'great! Fill it!'
Now the question is...can you trade it in at the pharmacy for the other company? I don't know and never heard of it being done, BUT...it doesn't sound so bad of an idea really. I mean if the quality isn't there, it's not your fault. So think about it. Sorry it happened, I really really am, because it happens too often.

I thought I was crazy!! I should be able to get another script from my doc and fill it at my regular pharm. I'll have to pay the full price as the insurance only covers refills when you run out...you know what I mean, but I'm sure my doc will give me another script. The regular methadone will only cost me about 30 dollars so it's not really a big deal.

Thank you so much for your reply. I have been reading anything I can find on the web and have found that methadose does not work the same as methadone for some people....(me included).

Oh - just to add on the refill issue, I live in a semi small town and have been seeing my doc for at least 6 years. He is a primary care and also a pain management specialist and will totally understand when I call on Tuesday to get another script for the "right stuff". He routinely gives me three scripts at a time when I call for a refill. I've always followed the rules .... no refills till I'm about out and this is why I'm in the predicament....I will be out of methadone today and my pharm is not open tomorrow cause of the holiday. I totally forgot about that and planned my refill to happen on Monday. My doc will give me another script no problem as I have been on pain management for three years with him and never had a problem.

I too, take methadone, but for my cancer pain. One time, just as you have, I got my perscription filled and it was mallincroidt (sp) methadose...definitely not the same! Shoreline is VERY KNOWLEDGEABLE about this. He gave his friend, who is a chemist, a Roxanne methadone pill (good pill), and a mallencroidt methadose pill (bad pill). Come to find out Mallencrodt....(I call them mallenCRAP, cause that's just what it is)... has less methadone. I think it was something like .08 mg of methadose. I could be off on the numbers, so don't hold me to it, but definitely less methadone. A Roxanne methadone pill has 10 full mg of methadone. I also felt a huge difference, luckily my PM doctor is SO KIND and on the prescription pad he checked the box for DO NOT SUBSTITUTE. Do yourself a favor....stay away from mallencrap :nono: ! Hope and pray you are all pain-free!

Mallinkrodt is the only bulk manufacturer of methadone in the country, so even Roxanne buys their bulk Meth HCL from mallinkrodt, It's just a matter of how much meth they put into each tablet.If they can make 12 tablets for the price of 10, you can bet they are going to do it. It was my brother that tested Methadose for me, He was the quality control and the FDA liason for another major pharma company. The 10 mg Methadose tablet I sent him contained 8.2 mgs of meth HCL. I didn't have a Roxane tab to send him at the time.

The FDA allows a -20% or +25% allowable variance in the genric drugs. This keeps it close in qualities and strength to the name brand but if one generic manufacturer is using +25% and the other is using -20% your talking about a significant difference between generics. Both times I was given methadose brand I experienced withdrawal. Fortunately the second time the pharmacist swapped it fout for the Roxanne the following week when his order came in. The pharmacist must know something if he's willing to bend the rules that much.

If you start with methadose and stick with it, you would never know the dfference, but changing from one generic to the other you could have as much as a 45% difference. :eek:

This is the FDA definition of equivalent.
Approval of a generic version of a proprietary drug by the FDA requires demonstration of “chemical equivalence” (similar quantities and availability of the active ingredient in proprietary and generic formulations), and “bioequivalence” (defined by absorption parameters generally falling between 80% and 125% of those obtained with the proprietary agent under the same testing conditions) (2) The use of the –20%/+25% rule is based on a regulatory decision that for most drugs that difference in concentration of the active ingredient in blood will not be clinically significant.

They have never had their meth dose dropped 40-45% to know if one generic was equivalent to another, they are comparing generic to Name brand, not comparing generic to other generics.

The profit margin is so much higher on generics, pharmacist are paid a bonus based on their percenatge of generics dispensed each month. This is why they continually tell us there is no difference.

Thank you all for your replies. Shoreline....you are just such a gem to have on these boards!!!

I'm finding that I need to take 3 methadose pills in order to get the same relief as taking 2 10mg methadone HCL pills. Have a call into the doc to discuss this....

I guess this is what I get for going from my favorite "mom and pop" pharmacy...the one that seems to care about my health to a "big box" pharmacy who doesn't know me from adam..... :o

Everyone is completly right. I was on METHADOSE when I lived in Little Rock,Ar. However, when I moved to Portland,OR my insurance company Kiaser Permanente, which has their own doctors and clinics put me on Methadone and I can tell you there is a HUGE difference. I don't like the idea of the FDA letting these generic medications slip away with a certain percentage less than the regular med. It makes a world of a difference, especially with pain meds..

Methadone 10mg=10mg methadose 10mg=8mg so the 40mg is only 32mg
