Can the general public schedule an appointment with a specialist or does a general practice doctor need to refer me first? I am used to having physicals and prefer to skip the routine and just see a Gastroenterologist.

I think it varies from country to country but if you have the mullah a specilaist will always see you directly of course. If you are hoping that the costs will be covered by your state health insurance you will need a doctor's letter (this is in Germany where I live but as I say I expect each country is a bit different.)
If you have cash to grease his/her greedy palms, a specialist wouldn't care a hoot if you have never seen a doctor in your life, if you excuse my slightly jaundiced view of orthodox medicine.

I think it all depends on the type of insurance you have the proceedure as well at the specialists office. I would just call them and ask...
