Hi there!
I have not had any hip replacements but I do have hip pain so I'm wondering if ya'll can help me?!
My recent hip xrays say this: There is appearance of flattening of the femoral head medially. No appreciable joint space or sclerotic change. The acetabular margins are otherwise normal in contour. No evidence of fracture or dislocation." This is for both hips but as far as pain, my left one hurts worse.
Can anyone please, please tell me what this means or why I have such bad pain from this? It does bother me to walk but the absolute worst is after laying down or sitting, I get up and then I have to take about 5-6 limping steps in pain before I can straighten out & start walking regular.
Orthopedic really had nothing to say except maybe there could be some inflammation and that the flattening is normal. But then I read somewhere else that the flattening is like bone touching bone.
I'm so confused & hurting....if anyone can help, I will really appreciate it so much!!!
Side note-----I have a bulging disc at L5-S1 and current xray says "transitional anatomy at L5-S1 involving the interarticularis" AND I also have my right leg about 1 inch shorter than my left! Do these have to do with my hip pain????

Roder, Welcome to our board. It sounds like you have a lot going on that could be causing your pain. Did the Dr tell you that you have no cartilage to cushion your hip joint? If this is the case, THR would be your only option. Maybe you will have to find another Dr that will give you a more definitive diagnosis and what the treatment options are. Having such awful pain is so debilitating and really effects your quality of life. I hope you keep posting and update us on your condition. Good luck....balleteach

Just a quick thought here; my x-rays did not look all that bad. It showed some joint degeneration but not anything that would indicate needing THR. It took MRIs with contrast to show just how much damage there was and it took a very skilled surgeon, not just the radiologist, to tell just how bad they were.
As it turned out, my hips were even worse than my surgeon thought they would be and the pathology report came back 'severe osteoarthritis'. No wonder I hurt so much for so long!
I also have spondololythesis in L4-5. But since my BTHR, my back has not bothered me at all. Spine and hip docs think I may have some relief since I am walking better now. Sure hope they are right!!
I think if I were you I would get a second opinion and ask for an MRI with contrast. May show lots more!

Thank you both, for replying to my post! I've been sitting over here feeling like I do want a second opinion & I would like to have an MRI since I know they can show so much more but of course that one orthopedic said no. I'm going to call ins on Mon. to ask how I go about getting a 2nd opinion.
Are you both considered young for needing your hip replacements? I'm 42. What kind of reasons cause the hips to go bad this early? I was always active growing up and have only slacked off in the past 4 yrs.
Another question if you don't mind!......did you get leg pain after standing? I get leg pain a lot if I'm continually on my feet and the other day I stood talking to a friend for 2 hrs at a store and OMG, I had to go lay on the couch afterwards!!

Roder, I have no idea what flattening of the femoral head means. Ask your Dr about the difference between that and loss of cartilage. Maybe he can clarify the difference. It seems like the ortho would have mentioned a problem with cartilage if that was the main problem. It all sounds a bit unclear. But in the meantime, you are suffering a lot of pain. I hope you can get to the bottom of the problem, so you can get it fixed. If you go to the Bio thread, you will be able to read the histories of some of the hippie hipsters. You will get some good background about us. All I can tell you is I was in terrible pain, limping, and it was excruciating for me to get in and out of my car. After the THR and recuperation, I was in great shape and the hip was pain free. Check out the "six months after hip replacement" thread to get even more info. I am considered young for THR but I am a tad older than you. My bad hips are mostly inherited. My hips are antiverted with internal rotation. Plus I used them long and hard.
And yes, standing still was very, very painful! Just waiting in a grocery line was agony. And when I had to stop to talk to someone I was always squirming and shifting my weight.
The first OS I saw did not tell me I was bone on bone either. He just said I had some arthritis in my joints. I went for 2 years trying every alternative treatment there is. It was only after my first OS sent me to The Most Wonderful OS to see about an arthrogram for what he thought was a labral tear, that I got any help. TMW OS took one look at my MRI and told me there was no cartilage left to repair and both hips would need replacing within a year. Now I am 6 weeks postop and already so much better than before it is hard to believe!! Makes a big difference to find the right doctor.

Hi,
I'm wondering if you use a lift to correct your 1 inch difference in leg length. If not, that would be the place to start. I have a 3/4 inch difference since my second THR and the lift makes a big differnece in my pain level--I have very little now.

Legtoolong...guess I know why you picked that name~~haha! I used to wear a cheapy shoe lift that was made for $30. But it wasn't good enough; nothing permanent. I mostly wear flip flops & sandals so it got to be a bother for me & I haven't used it in forever. I really wanted that orthopedic doctor to point me in a direction to check out this difference but I guess I'll just have to go get a 2nd opinion. I'm pretty sure there's a connection between my hip pain & this short leg. Of course I hope that's all it is, that way I could avoid any surgery, ya know?

I still have hope after all these years of struggling to find answers, that I will get better...and I'm the world's biggest pessimist!! I also constantly shift from one leg to the other when I'm standing....you don't need to do that anymore since your surgery or is it too early to tell? I was telling my mom some stuff and she said she thinks my dad's hips are bothering him. She said he gets out of his recliner very s l o w l y. I told her to ask him for me so I could know if there is a chance of inheriting a hip problem.

I wish I was more bold during my last appt but I was trusting him as the specialist; I should know better to trust myself & what I am feeling! I mean, he didn't even offer to send me to physical or water therapy. I'll also check into some of the older threads.

A friend of mine who is an anesthesiologist and also has a pain specialty always checks people who come to her with leg, hip or back pain for leg length difference. She has told me not to go barefoot or wear anything without my lift. Often this solves their pain problem. Her patients think she is clairvoyant when she asks if they have had hip or knee surgery when they complain of back pain. She usually finds a leg length difference.
My surgeon tried to make light of my length difference and only prescribed a 3/8” lift at first. That did not work and left me with a strange gait which my friend looked at and said I needed more lift. I went back to PT and worked on the gait and the therapist helped me get just the right amount of lift. He says it is a trial and error process between what looks right to others and what feels right to you.
Hope you can find another doctor who will listen and know what to do.

Thanks for the post Legtoolong! I do have my doctor working on getting the referral for my 2nd opinion with an orthopedist. I may schedule a chiro appt in the meantime bcuz I'm tired of waiting with this pain. I hope the 2nd ortho can tell me of where I can get a good heel lift...I wonder if I should start wearing my cheapy one til then?? It looks so stupid tho bcuz I was taping it to my flip flop!!
Anyway, I hope all of you are doing well! I will be coming back to check in when I have more info on my problem or to find more info from you guys.

I use two sided mounting tape to keep my lift in my sandal. Look for lifts on the internet. I found some good ones with peel off layers to get it just right.

Hi Hip Pain - boy, I can relate. I had hip pain for 10 years which was diagnosed as bursitis, a bad back, one leg shorter than the other as well as other things. I took so many anti inflamatory drugs that I now have acid reflux and can take NO pain meds. Anyway, I wore a lift in my shoe for 4 years and it did seem to help the pain but not solve the problem. I finally had a special MRI that looked at the hip in detail. They found that I had a cyst, a labral tear and lots of "gunk" that needed to be cleaned out of the joint. I jumped in and had a hip arthroscopy last September. My orthopedist wanted to perform the surgery but I opted for Dr. Sampson in San Francisco because he is one of the founders of this procedure.
My pain was relieved immediately. Dr. Sampson did find that I had flattening of the femoral head and he shaved down the ball at the end of the femur in order to fit into the ball without pain. I think the name of my condition was "femoral acetabular impingement". My hip is much better now and all I have to show for it is two small 3/4" scars on my left side. I even had the surgery with just an epidural which meant I was awake during the surgery - just don't recall any of it due to their wonderful "forget" drug.
Anyway, the past 3 weeks I have been in severe pain due to my back locking up. I have been going to accupuncture (which, by the way is a great way to relieve some of your pain) and the pain has now localized to my right butt. I went to the doctor and was told that it probably is a labral tear in my hip (this is the hip that has not been operated on). I am going in for another MRI to see what the cause is, but may need to have another hip arthroscopy on my other hip. What is the chance of that? It could be that my hip problems are a bit congenital.
In summary, I would definitely recommend that you have the hip arthroscopy performed by a doctor that has quite a few under his/her belt and has been trained by one of the founders of this procedure. Any questions, just post to me.

I just came back to the board to read up as I have my 2nd opinion appt with the orthopedic on Tues the 16th. Do you know if there is a specific name for this special MRI you had of your hip? I want to talk to the specialist about doing some type of MRI of my hips before I begin any physical therapy (which he will need to get me a referral for).
I know it seems weird to say this but I would welcome a cyst or a tear....anything that is obvious just so we can say "ah-ha, that is what has been bothering me" and have surgery if needed and get away from this pain!! Seems all I ever hear when I have things show up, is...."well, it's mild" or "well, that wouldn't bother you"!

P.S. Can I ask you a question?....Since you did have flattening of the femoral head, I want to see if you had any pain by just pushing in on & around your hip? I'm trying to figure out why that would hurt me so much to push in there (only the left one does that). It seems like it could be the underneath/inside of the hip bone???? I saved a few websites on my favorites for lifts that have tape. Some look pretty good & some seem expensive since they say to replace after 3 months from wear! Well, I'll write more after my appt.

The tape I used was Scotch brand from the drug store--much cheaper than others I saw on the web. It is for mounting things on the wall.

Hi Everyone, I am new here and have been reading all the posts. I just went to the dr. last week because I'm having terrible pain in my right hip. Most of the pain is on the outside of my hip, but, I have groin pain also. My dr. thinks it might be avascular necrosis and has ordered an MRI be done. He thinks it is from taking prednisone for 14 yrs. for addisons disease. I am in a great deal of pain, am limping, and this has generally put a real damper on my life. I am 52. He also mentioned something about hip replacement. Any information or words of encouragment would be much appreciated .

The MRI seems to be the best information they can gather, so I'm glad you are getting that. Your pain sounds a lot like mine before I really knew what was wrong. Have you tried walking with a cane for some relief? I was sent to PT before I went to the Orthopedic surgeon and the only good it did me was that the PT got me using a cane. He said to use it on the opposite side of the pain which didn't seem logical at the time but it works that way. He said you get 30% more support using it opposite the painful side. Don't let vanity stop you from using it--I was reluctant at first, but it really helped me, and I looked better with the cane than limping.

Did he check to see if you have flat feet?
I am 76 years old. I have a misaligned frame from a childhood accident. I developed flat feet in the last 10 years. Four years ago my right hip got to hurting and for a while I thought that I was going to be in a wheel chair shortly. Xrays showed moderate arthritis.
I was taking glucosamine. It helped, but not enough. Then I also took flax oil, and that helped a lot. But then, - I got arch supports!. I'm climbing stairs again, I walk the mall etc. And I take bone meal as a calcium/mineral supplement. It is the most absorbable of all forms of calcium.
It is really frustrating that the doctors are so eager to do surgery, and nothing to help you get well by yourself. Of course, some folks just wait too long and get past the point of return.

Hi, Legtolong, Thanks for answering my post. I've bought a cane, but, keep forgetting I have it and leaving it places! I will try using it on the other side. My mri is this thursday. My 7 year old son has the same problem, he has been diagnosed with Perthes Disease. Which means that at some point in time he lost blood flow to the hip socket which caused the femoral head to deteriorate. There is a lot of information out there. I would check it out and see if it sounds like you might have this disorder. Take care.

Hi!
I sure am glad I found someone to talk with. I'm writing about my mom. She is 70 years old, not in great health, and is need of a hip replacement according to the doc. The problem? My dad has Alzheimer's and he will freak if she is not there for starters.
Her pain began in April 2005, so it hasn't gone on that long, but it is immobilizing her. She can hardly do anything. I do all of her errands, etc., and I really can't afford to take 6 weeks off work right now to care for her and my dad together. She had a cortisone injection, but it only last 2 weeks.
I've been reading about prolotherapy, is anyone familiar with that? I'm just trying to find some alternative to hip replacement surgery to relieve her pain. Can anyone help me or refer me to someone who can? We live in Knoxville, TN.

I sure am glad I found someone to talk with. I'm writing about my mom. She is 70 years old, not in great health, and is need of a hip replacement according to the doc. The problem? My dad has Alzheimer's and he will freak if she is not there for starters.
Her pain began in April 2005, so it hasn't gone on that long, but it is immobilizing her. She can hardly do anything. I do all of her errands, etc., and I really can't afford to take 6 weeks off work right now to care for her and my dad together. She had a cortisone injection, but it only last 2 weeks.
