This is what my ultrasound report reads. Something overheard about them talking about the adeno thing - "the uterus is growing into the abdom. muscle" or vice versa - can't remember? I know the Leio thing is my fibroids - I have 3. Doc says it's nothing to worry about - I could treat with the pill or face surgery.
Doctor said we needed to schedule an appointment later to discuss hysterectomy surgery.
Can anyone explain this to me further? Is this why I am bloated, cramping pains, and having a 14 day cycle this last month?
Also curious if I have the hyster. - should I have everything removed including the ovaries?

Adenomysis (or endometriosis interna) involves the growth of endometrial tissue from the uterine lining into the uterine muscles. This disease may coexist with external endometriosis in which endometrial tissues are located outside the uterus. The abnormally located endometrial tissue tends to bleed with the menses and the blood and debris may accumulate creating small fluid collections inside the uterine wall. It can lead to swelling and the uterus often becomes larger (i think the docs call it "boggy"). Adenomyosis may present as a diffuse condition or it may be focal. In the latter, there are local areas of swelling, so-called adenomyomas, that may mimic other uterine masses.

What I know from personal experience is that it wont go away and in many cases gets progessively worse until you are forced to do something about it. For me, i had flooding, extremely painfull cramps leading to fainting and nausea, bloated and anaemic. I tried bcp for years, d&c, mirena implant, laser treatment. Most of the treatments did work for a short while before the condition got worse. I often wonder what would have happened if i had never touched a bcp ......... who knows.
In the end, it was affecting my work, family, and i was just a miserable person with no enegy for anything. I was advised that a hysterectomy would be a solution to certainly the bleeding and probably most of the pain. I did have a 2nd opinion and also enquired about ablation but the original choice stuck. I also found out that I was one of those women that had a very small amount of endometriosis and adenomysos but even the smallest amount gave me the most excrutiating pain.
I had a TAH in April and am now feeling great. I still get a bit achey but the pain of the operation itself was less than the pain i had every month. I decided to keep my ovaries as I might get 5 years use out of them, 10 if im lucky. A lot of women opt to remove them as they say it can cause endo to come back. My ovaries seemed to get over the stress of the operation and I can tell when Im ovulating but dont get any pain.
I was also given the choice of keeping my cervix which would have meant having a SAH but it runs the risk of mini-periods and all that entails. It means they have to move your bladder out the way to take it out and brings with it more risks but I think it was worth it. There is also a school of thought relating to sexual response/desire following removal of cervix and/or ovaries. I have found no problems :D

I would certainly ask for a 2nd opionion. It is an elective surgery and invasive but I have to say that even though i did go for 2nd opinion, my gut instinct told me it was the right thing to do even before the doc suggested it.

Can you tell me what the abbreviations of TAH and SAH mean? I have had some cervical dysplasia in the past (prior to my 2 kids) and from what I am understanding I may need the TAH to have the cervix removed as well, right?!
I originally had asked the doc if this was fibroid removal surgery or hysterectomy and she said definately hyster and we would have to schedule another appt to discuss it more fully. Not sure if I need a second opinion or not - I had been seeing a gyn onc for the last 6 years because of the previous issue, but my paps have all been good ever since my kids. Prior to that I had the cyrosurgery the first time and a D&C and scraping (cone?!) the second time (about 9 months later) for mild dysplasia.
Since I've had my daughter in 99 - everything is so much worse - the cramping and the hemorraghing periods every 21 days. The gyn onc said I just have fibroids and has never once gotten an u/s - just feels for them every year for size. Even when I've went to him complaining about the excessive bleeding and pain. He is a VERY conservative doc since he has to always do hyster for cancer - he believes if a woman doesn't have cancer - she doesn't need a hyster. I know this cause I begged him for one in 2000, but he refused cause I was too young (32 at the time).
Now at 37, have been given this adenomyosis diagnosis along with the fibroids and wondering why it's taken so long to get someone to talk surgery with me. I'm tired of feeling bad all the time and wanting to crawl into a ball during ovulation and periods besides the fact that I have ruined so many clothes. I used to try and salvage them - now if they are nothing nice (which I don't wear during anymore), I just toss them cause I'm just too tired.
Last ferritin check was 5 down from 6 in Feb.
So, I first wanted to say thanks for the response and maybe I'll go with my gut instinct as well. Just have to get in enough work hours before I can schedule this which puts me into October. But I guess if I've survived this long - a few more months shouldn't be that bad, right?!
Well - I guess I'll be chatting with you more later as the surgery date approaches.

I am so sorry with what you are going through, and with a doc that wont listen. My OB ( well my olkd OB) was the same. If it is not cancer, no hysterectomy !! Needles to say I switched OB's.
My story goes like this. Since I was 13 always had severe painful periuods and irregular periods,was told after having kids it would go away. Never happened. My pregnancies were not good either.
18 years ago was put on Naproxen sodium for my pain. Also developed severe ovulation pain.
8 years ago I injured my back and had a MRI done. My back doctor said that the MRI showed a Enlarged Uteris and or a Mass on the uteris and I have to see my OB about that. I did. I was nervous !! My OB checked my out and said yes it is enlarged but no big deal.
I was put on the BC Pill. It helped the ovulation pain and got me regulated but did absolutly nothing for my period pain.
Last july I developed a 6cm cyst on my left ovary. You are not supposed to get cysts on your ovaries when you are on the pill. I kept developing cysts and it wouldn't stop, and all he did was constantly change my BC pill and said that the majority of the pain was IBS. He would not even give a Laporoscopy. He new something was wrong, Out of the blue he said I wasn't having any more kids. he also said he would give me a Hyst in 10 years. (I am 39 now) He said he didn't belive in a Hyst at my age. I was too young.
I just wanted the pain and cysts to stop. I saw another OB and as usual had more cysts and more pain. He gave me a Lap and D & C.
My utreris was Enlarged and Boggey, I had ruptured Ovarian Cysts, and Severe Pelvic congestion.
My choices were to either still find another BC Pill ( mine was changed 5 times in 10 months with no help) so that option was out,Lupron or a Tah Total Abdominal Hysterectomy. He said he could not promise about the Lupron working, but we would have to try to see what would happen, but felt the Hyst would be a better choice. I had the hyst. pathology confirmed Adenomyosis in the uteris.
NO MORE PAIN !!! What a feeling !!! NO REGRETS !!!
I did keep my cervix but everything else is out.
Get another opinion. It is your body. A woman knows when there is something wrong.

I had a TAH 4 years ago and i couldnt of felt better after it. i had the whole works taken out. i was bleeding to deth from my problems and the only way to stop it was to do a TAH.at 28 i have to have HRT in shot form cause pills wont work and im alarigic to the adheasive in the patch.
i had many serg to prevent me from having a TAH but unfortanly it didnt work.
i hope all goes well with you.you do what is the best for you its your body and only you know how you feel. good luck and let us know.

Thank you both so much for your encouraging words.
I have decided to go for the hysterectomy, but now my concern is this doc's opinion about the surgery.
She sat down with me and hubby on Friday afternoon to discuss everything and I showed her pictures from my previous doc's hysteroscopy that showed I had tons of endometrial varicosities (varicose veins on the inside of my uterus) and told her that he said he had NEVER seen a uterus like this in all his years of practice (and he's a gyn oncologist). Well she read through the report and said this should be interesting. That she would do a Total Vaginal Hysterectomy and that she would only sign me out of work for 2 weeks.
WHAT?! A 2 week hysterectomy - has anyone ever heard of this? First - I want someone to at least check out my ovaries while I'm knocked out and that would require the laproscopy assisted TVH. But I have never heard or seen written anywhere that someone could return to work in 2 weeks! Am I missing something here?

A 2 week Hyst does sound quite fast !! You are having it done vaginaly ?? I had mine done the old fashioned way...I was cut.

I don't know much about having it done vaginaly, but it still sounds to fast. I was back at work at 3 weeks. Only part time and only a couple hours evey other day. I have a desk job, so that helped. I also work with my husband.

As for your ovaries, if you are having problems with them I would deffinately talk to the doc about having a Lap done first.

During my Lap my ovaries had ruptured cysts. Then a month later I went in for my hyst. My doc was debating about taking the ovaries out, but when he opened me up when I went in for my hyst, my ovaries were enlarged and cystic, so needless to say he removed them. My doctors think that I was also PCOS and I am still getting treated for it.

Talk to your doc about a Lap...It maybe worth it !!!

Dear hrt

A total hysterectomy means removal of the womb..... your uterus and cervix ...... these are major parts of your body and their removal means disruption to the remaining parts.

A vaginal removal is less invasive ..... no cut = no wound but only on the outside. The surgeon will have to do some snipping inside, probably move your bladder out the way and make sure your colon does not prolapse at the same time !!!!

In my opinion, you will probably heal more quickly from the wounds inflicted BUT IT WILL STILL TAKE A GOOD SIX WEEKS for the rest of your insides to get used to the new you. If you return to normal activities too soon you run the risk of infection, prolapse of colon and bladder dysfunction.

Research some more, arm yourself with some pamphlets or print outs and see your gyno doc again.
