Hi, I am having a long saga of problems with this molar.....It had been sensitive to sweets for years....Felt a little worse last year and had pain on biting down so I had it crowned (long story, should have been done earlier but I had an incompetant dentist)....My new denitist sees no cracks or abscess on the xrays.....I now have permanent crown on with temp cement since I have been having an on and off ear ache in the ear on that side for over a month.....He adjusted my bite, the crown and put some anesthetic stuff on there because he doesn't see a r.c. problem. Although I have read many times that sometimes you can't see the crack on xrays. I am still having the earache which is more noticible at night. I havae seen 2 ents and my ear is fine......I have made an appt. with a endodontist for a consultation in about 3 1/2 weeks but am thinking of trying to move this up.......I am favoring the other side but do eat on the bad side to see what the pain is like.....I have sensitivity to hot that can linger for minutes(though not every time) and seems to travel through about 3 or 4 teeth to the front teeth which makes it hard to pinpoint but I read the nerve from the bad tooth can make the discomfort travel.....I don't have the intense pain on biting down though I haven't bitten on anything like a rock because I am afraid of doing more damage on that side. I don't have much pain without eating, although I did a few times and very mild. I do have tmj but have never before had ear pain from it.........So, I don't know what this is , do you think the endo will be able to tell? Do these symptoms sound like a root canal case? Are these mysteries common?.......I do not like the idea of a r.c. at all as I am very anxious healthwise and even the though of the epinephrine in the novacaine is frightenning me.....Please give me some advice/answers......Thank You, Maria

If there is a dark area at the end of the root, a root canal is indicated. The absence of a dark area on the radiograph does NOT mean that no root canal is necessary. It takes somew time and almost 40% of the bone to be destroyed before something shows up on x-ray. Lingering pain to hot (with earaches) is a pretty good sign that a root canal is indicated. I would suggest moving your appointment with the endodontist us sooner if you can.

Anyone else? Are there any other dental boards with more action that you know of, I don't get many replies to my posts specifically about my question and not generally about the subject (not this one, my one answer was helpful) but at other times....Thanks, Maria

Ditto getting in to see the Endo, pain to hot is a sure sign that the nerve is irreversably inflamed.

I've still got 16 days to go, I couldn't get a closer appt....I am also wondering about swithching to another dentist since I told him it was sensitive to hot and although I thought I saw a light go off, he still said he didn't think root canal at this point. I called him back after about a week when the pain, sensitivity didn't go away and left a message, that was over a week ago and I have not heard back. I feel horrible, I liked the guy but this is not right..He doesn't even know I'm going to the endo but I need a real diagnosis, I can't guess at my own..Maria

If a cavity is deep but still not into the nerve, this can still cause many discomforts and sensitivities, so none of us can advise you on what could be going on: we cannot see your teeth. But I would suggest to you a cold pack on the painful area if it becomes a little more prolonged before your Endo visit, as you've not mentioned a sensitivity to the cold.

Pain upon a hot sensitivity can indicate nerve damage such as infection/abcess, etcetera, but also very little, like inflammation.

Thank you, it seems my dentist was not very good at getting at what is causing this so I pray the endo will figure it out. This tooth (and I can just assume it is the one causing the discomfort) already has a crown on it so there is not much tooth left for a cavity and the dentist saw no cracks or cavity or abscess on the xrays) Don't you hate when they can't get to the bottom of things or it is not easy to figure out? When you are an anxious person this is hell... The strange thing is, the only problem I had with this tooth was pain on biting until I started having the (2) crown work done on that side. That is when I got the ear ache , jaw ache, and the sensitivity and intermittent tooth (teeth) aches. Thanks, Maria

Sounds like your tmj has really flared up! I have tmj and I have pain in ear,temple and jaw. You are probaly grinding or clenching your teeth at night. Do you wear a nightguard, if not ask your dentist about getting one. Good Luck!

Hi, for anyone who is following with interest or if anyone would like to comment, here is the update on my problem.....I saw the endo..He took (yet another) x-ray..Tested for hot and cold and tapped around.......Nothing! Don't ask me why I had no sensitivity to hot when previously when I was home it and held hot liquid on that side it hurt! He doesn't want to do the root canal......No signs on xray etc.....He said it could heal itself...My dentist is not sure either.....So for now I am babying that side. Using senstive toothpaste and I'm having a bite guard made..Something is not right on that side of my mouth but no one seems to know what......I've been told it will either get better or get worse......No kidding.....That doesn't help my anxiety......So for now, I am back to square one and trying not to worry too much.......Any replies are welcome.....Thanks, Maria

Also wanted to add that that side feels weird at times like it is very slightly wanting to numb......I feel a pulse now and then, usually for a few seconds at night when I lie down....the entire molar section (since I can't pinpoint it) is uncomfortabel to biting,not a sharp pain just more like an ache from overuse (which I don't). Ear still has problems though much better and hardly noticible...Can feel something from jaw joint to jaw/gum area and to wear jaw attaches to bottom of ear....This is so hard to explain.....Am I the only one with this mystery problem? I want to get this fixed but if my dentist and an endodontist (that I chose because he had more training than necessary) can't figure it out,,,,what do I do?? Please help....Thank You! Maria

So weird this sounds excatly like what i'm going threw right now , I had 2 fillings filled (one one week one the next )had a horrid ear ache on that side (they where the 2 back molars )then woke up 2 days later to the worst pain of my life (and I have dislocated my shoulder and gone threw 28 hours of child birth)i though i was going to faint everytime a wave of pain hit me .the best the dentist (not mine 0 could come up with was thet I had an abscess on a nerve under one of my teeth but he was unsure of wich one ..taping them didn't seem to tell wich one .So he sent me home with tylonlo #3 , penacillin and motrin 600mg .........and here i am aweek later and it'still hurts but not as bad . He said my grinding my teeth made the abscess irritate and i had to remember to wear my mouth gaurd or it will get worst .I never want to feel pain like i did last week ever again .So Mariazzz did they ever find out exactly what was wrong ...........i'd really like to be better by xmas !!Or Sooner!

Greetings...
Sounds interesting. Abscesses don't always show up on x-rays. My jaw ached and ached and ached. My ears, both of them even though one side had the problem, were ready to drop off, my cheek would numb, malaise etc...endless list of symptoms. No ddt wanted to do anything as nothing was visible on panoramic x-rays. I had a feeling the pain and malaise was radiating from a rc tooth that had most probably failed. Every ddt tapped on it and nothing...until I finally had it pour into my bloodstream. It was great "fun." All of my glands started to swell and I felt "poisoned." Still, no ddt believed me that my rc may have failed until I came to one older, more experienced expert who said "out with the tooth." Lo and behold, at the bottom of one anatomically complex root was a deflated white vac. Bingo. I don't mean to scare you, but unfortunately I was one of those "fortunate" few 0.01% of patients that ends up at ER from a tooth abscess that nobody had detected in time. Since my toxic episode in the summer, I continue to have ongoing problems with all of my glands...apparently the one year dental issue has woken up a "sleeping" bug in me. I am also prone to anxiety and my immune system has taken the toll. All that bacteria and stress is a recipe for disaster. Often back teeth ache when there is something locally affecting salivary glands. When they swell they put pressure on the lower back molars. If you haven't got an immune problem, viral or bacterial infection from another source, then most likely, your glands are locally reacting to a dental problem. Have you still got your wisdom teeth? They can also do wonders to your jaw, ears and glands. I had to wear a night guard for a while thinking I had TMJ...but TMJ is often an excuse for a ddt when he doesn't know what else to say. TMJ is often caused by abscesses and wisdom teeth if you haven't got a malocclusion.
Anyway, just some food for thought. You know your body best. Don't let the ddt send you to a shrink...I was sent twice even following my shock (who would want to make that up????I am not a hypochondriac). Just find someone who will take you seriously. I finally managed to find an ENT who is taking my problems seriously now and helping me get gland biopsies and sonograms done...unfortunately after xmas so I am also worried about getting thru more pain and malaise and the occasional chipmunk look when my glands decide to inflate.
