View Full Version : [Help] Getting hurt inside a store...

What form do you have to have the store fill out if you got hurt inside a store? I looked on google but only found the forms for employers and employees. I found a form for something similar but it's for another country.

The person I spoke to inside the store said they don't have any accident/incident report forms, she said I can get a copy of my records.

Does anyone know which form I'm supposed to have her fill out?

BTW In case you're wondering what happened. I was trying on my new contacts, and the assistant gave me a bottle of hydrogen peroxide instead of contact solution...

edit: I had witnesses, the thing burned like crazy and the doctor had to do an eyewash on me, checked my eyes 9324809324 times, before I could go home...

now my eye is hurting and I wanted to know what I can do about it... I have an appointment with the optometrist tomorrow, so I'm trying to find the form tonight.

Oh and I didn't know about the accident report form until someone told me I should have gotten one... so it happened a week ago, but they DR said she would fill it out.

Help me please... Thanks!

I'm not sure where you can find this type of form. But I would insist to speak to the store manger and get this taken care of as soon as possible. They ARE at fault and have insurance to cover this type of thing. You may have some long term damage from this incident. Its up to them to provide the forms for you to fill out!
If you get no results from the manger then I would tell them you will be speaking with a lawyer!

I'm thinking the same thing. I've worked in sales and it is policy that they fill out all accident reports and that you get a copy of them. You should go back to the place of the incident and demand a copy. I don't know of any place that doesn't fill out and have any accident reports.

I'm just wondering where on earth they expect you to find and accident form. It seems that this place of business isn't doing what they should be. And like gardenandcats stated, if you get no response or help from the manager seek the advice of a lawyer.
