You need to see a urologist. Even then it will take time for the urologist to find the right medicine for your particular infection. Did the doctor do a culture to find out which bacteria you have? For severe prostatitis hospilization and IV antibiotics are used.
It usually takes at least 4 weeks of antibiotics to take care of an infection. These days with so many bacterias becoming antibiotic resistant it may take longer to find one that will work.
The severe pain is most likely caused by a duct from the prostate being clogged. A warm bath and then a light massage might help open the duct.
Having sex will help keep the prostate drained, but, what wife wants to have bacteria introduced to her. I have been down that road with my husband and the uretha infection I suffered was severe and difficult to bring under control. Doctors will swear up and down the infection cannot be transferred to the wife but from experience I can tell you it can and will be.
Swelling will cause more pain, constriction will keep urine from passing freely and can back up into the kidneys.
Sometimes surgery is necessary to remove part of the prostate in order to relieve the pressure on the bladder, etc.
AVOID caffeine and spicy foods. Take a stool softener or laxative to prevent constipation. Constipation will increase problems/pain, do not want to strain.
From my own personal experience with bladder and uretha infections AVOID carbonated drinks. Drink plenty of water.
In my opinion 3 months is too long to wait. If you have a medical school in your state or other, head there and get help. Professors at medical schools also have private practice at the school and are more up to date on treatments. In Texas go to Southwestern Medical School in Dallas, Texas.

Sounds like you could be having bladder spasms and there are meds to relieve spasms. Ask your doctor for a med to relax the bladder. Also over the counter, think it is still over the counter, Predium. Not sure if I spelled that right or not. Predium will turn urine ornage, so don't get shook. Predium helps relieve bladder pain.
When I had a bladder infection the doctors kept saying I did not have an infection. Unfortunately I had an infection in the glands in the uretha at neck of the bladder and it did not show up in urine tests. Antibiotics did not touch it since that area does not get direct blood supply and nourishmet comes from adjacent tissues. Not sure if the same thing could be for a male or not. I can relate to the pain you are experiencing.

Drinking "real" cranberry juice, not the watered down version, is suppose to keep the acidity of the bladder balanced. If you cannot find real cranberry juice, eat cranberries.
