My doctor wants me to keep a headache diary for a month. What all should I write in it?
Date & Time of Day of Migraine/headache.
How long did the pain last?
How severe was the pain on a scale of 1-5?(1-least severe; 5-most severe)
Where did you feel the pain?
What type of pain was it? (Examples: throbbing, stabbing, dull, sharp)
What may have triggered the headache/migraine?
What was the method of relief? Was it successful?
The above is just an example of a Migraine Diary. Other examples can be found by doing a Google search. In the search field just type the words migraine diary or headache diary.

That's great. New Dr.'s or new patients can get alot out of a headache diary. I started keeping one in 1977 and for the first 5 years or so wrote in it almost every day.
That was enough to pick out the patterns that my headaches took. From then on I would write once or twice a week and mainly if anything was different.
A couple other things to include are;
What was the weather like when you got your migraine.
What time of day was it and what were you doing at the time.
Keep track of every bit of medication both Rx'd and OTC.
He will probably want you to try a food elimination diet sometime in the future so you might kill two birds with one stone and keep track of what food you have eaten.
