It sounds like you have muscle tension in your neck that is causing your headache from the way you describe it. 20 years is a long time...surely you have been to a doctor? There are several different medications that might help you, but you would need to find the reason for the headaches, like is it emotional stress, physical stress, pinched nerve, chemical imbalance, tumor, etc., etc. Maybe you need a spinal adjustment (chiropractor) or a massage? Why do you think you have these headaches?

david mccoy...
I had horrible migraine-like headaches for almost two years, day & night. Tried every prescription and OTC medicine possible. Finally insurance agreed to pay for an MRI of my brain. Surprisingly, a sinus infection showed up. A CT of my sinuses showed that both spleniod sinuses were swollen closed due to infection.
I had NO symptions of sinus infection. The splenoid sinuses are behind the eyes and are not involved in breathing. I did not have a stuffy or runny nose. Outpatient surgery was done (through my nose) to cut a "window" in each side. I have not had a headache since. My temples but especially by upper neck was where the pain was worse. Since your pain is also in front that may be a clue that the spleniods may be involved. The only thing that helped me was to fill a bucket with ice water and freeze my head. Hope this helps.
Good luck.
