Can someone tell me what the IVF screening process consists of. As far as: what exactly do they ask, look into, what are they specifically looking for, what would disqualify me, are there stipulations based on income, etc. etc.
Any info you have would be helpful.

Well, I don't know if you can call it a "screening process" per say, but I'll tell you my experience. My consult was more on the informative side, after IVF with ICSI was recommended. That was of course, after questions were asked.
Some of those questions were: how long had we been trying to conceive, tests we had done so far, and some questions about our past, like STDs and surgeries. Since my DH had had 2 SAs to look at, the doctor determined the best route from that. (He has had another since.)
They never asked about our income at all. They did a courtesy call to our insurance to see what was covered. Since that wasn't an issue for us, we never talked to a financial consultant. They did have them at our clinic though, as well as a program for payment options.
I'm not sure what wouldn't qualify a person for IVF. I think many couples go through it for a variety of reasons. Some get to IVF b/c other methods failed, so you may have to or decide to do others as a "precursor" to the IVF, and sometimes your insurance also requires that. It really depends.
