Test values are different from one lab to another. With diabetes in the picture it could be a cancer or it could be an immune system reaction since often diabetes is thought to be an irregular immune response to the pancreas and insulin making; that causes diabetes. 13 is not too awful high a blood count, but the B vitamin problem is a common one among people who have an auto immune disorder. The doctor is the one to figure this one out.

If her b-12 levels continue to be low while taking the pills then her doctor should be giving her b-12 injections monthly. You develop pernicious anemia because of abnormally low b-12 levels. If the b-12 level is corrected then the anemia should resolve.Clubbing of fingers is related to poor oxygenation of the tissues which could also be related to her anemia. Your sister also might ask the doctor if liquid b-12 might be better absorbed.
