I would like to start a pain journal for my doctor and was wondering if anyone had a template or know where I can find a picture so that I can color in my pain...similar to the ones that they have at the doctors office. Thanks in advance!!!

Purdue Pharma, the makers of OxyContin have a website called partners against pain. If you go to "healthcare professional" they have some forms and pain diary that you can download.

The American Pain Foundation has alot of really good downloads along with a detailed pain journal and alot of really good info, too.

Most doctors also have forms similiar to the ones you can download online that they will freely give you. The problem with most of the ones you see is they limit what you can put in it. I created my own with a database (but I'm a nerd) and my husband is writing a pain program that we intend to give out as freeware. It would be great if the moderators would allow me to post a link when it was completed because it will be free but I don't see that happening due to rules. Must respect the rules :)
You should track not only your pain levels but your activities that may have caused the pain, when you take your medication and it's always nice to have a spot to put any special notes and side effects.
Keeping documentation is great.
