When one speaks of foamy urine, just what amount is considered enough to be concerned about?

The foam is usually caused by protein. You can't judge from the amount of foam how much protein there is, so you have to have your urine tested. Typically urine without protein doesn't foam (or maybe a teensy bit if the stream is very strong).
