Would heart attack or heart failure didn't show cardiac enzyme elevate?

Some Dr will base on EKG and cardiac enzyme to detemin it is the case of heart attack or heart failure or nothing wrong. Well, some just base on cardiac enzyme only. If it is the case, how accurate or reliable it is?

A complete diagnosis is the result of more than just one test, symptoms, and clinical findings. An EKG is not very reliable as there are several factors other than a heart problem that will produce an abnormal EKG. For instance drugs, medication, electrolyte deficiency (potassium, calcium and magnesium) etc.
