I went to see my anti aging doc for a follow up yesterday on my blood work. He said something that was odd. He said that based on this

SHBG 11 (8-46)
T 464 (241 - 827)

that based on my SHBG of 11 that my T was fine. In fact he said Total T should be based on 30:1 ratio of SHBG so in fact he says that my T could actually be 330 and still be ok. He rec that I lower my dose of AG.
Has anyone ever heard of this? 330 is really low IMO and dose not seem like a good target for TRT...

The chart that Dr Shippen gave me isn't quite that linear. However, using the 11 number and using his chart would put 330 just inside the normal range. 500 is the midline of normal range, and 750 would be the upper extreme.

The explanation is that the anti-aging doc is concentrating on your free T level. The lower SHBG is the higher free T will be given a fixed T level. That is why he says you can be that low and still be "ok" meaning free T is still in the normal range. However, it's really not so simple. If you are still having symptoms of low T then you may need a higher free T level then the low end of "normal". Also, it is not uncommon for men with low SHBG to have additional problems. I have posted before that every low SHGB can mean other hormonal issues are going on that can effect libido and cause other symptoms of low T. Make sure you have your thyroid checked and are treating any TSH above 3 aggressively enough. This can add to the problem eventually causing insulin resistance. It is not uncommon for men with very low SHBG to also have thyroid problems.

My TSH is .94 I have the blood work up. What other thyroid checks should I look at...??? I posted most of this stuff in a thread called blood work about a week ago.. based on what I posted and what you told me, Hayfarmer, Insulin resistance was not an issue. I know this Dr also treats Insulin restance also know in sort as syndrome X I believe, and he said there was nothing to be alarmed about.... My Triglycerides where high and he said lower the carbs and raise the protein and start taking EFA's; Fish Oil....Saw Palmetto for the slightly raised DHT....

Yes I remember that. You can check T3 and T4 but they are likely ok. I agree with you though that the total T level he thinks is ok is likely too low. You really have to go by how you feel more than the numbers. My SHBG is 17 and based on Dr. Shippens chart Lowesclub mentioned my free T is about 225 pg/ml, my total T around 750. So for me I need to be at the upper end of the normal free T range. For you with SHBG of 11 that would correspond to 650-700 total T. You could try the lower dose to see if you feel better on it for a couple weeks but my guess is you need to be higher, not lower.
