Hello Everyone.
I am new to this page and I have a question about the form Adult Function report. I got mine in the mail and started to fill it out and have made a huge mess I couldn't fill it out all at once now the question is can I use a form that was printed off the internet or should I ask for a new report to be sent to me? I am so overwhelmed with this process I don't kmow what I am doing, any help would be great. Thank you, Gizmosmom

You should probably print it off the internet because I believe there is a time limit to get back to them.

Id try to send it in anyway or call your nearest SS office and ask them to send you another one Im sure they will

maybe they'll get so confused you'll get approved { my sorry}

I read of a person that filed for SSD it took them 6 months to fill out the paper work !
