The problem you probably have is called frozen shoulder. It is common in people with uncontrolled diabetes. Cortisone is often used, but it only masks the problem and will make your sugars even worse. Physiotherapy, with heat and manipulation, will fix the problem. For a permanent cure, you will need to get your blood sugars under control. Have you thought about a pump? What sort of regime are you on. There are quite a number of articles available about the theory behind "brittle" diabetes. I personally don't believe that there are very many people who are truly brittle. Very often it is simply that you haven't yet found what works for you, and someone to help you implement it.
Good luck.
